IT REQUIRES YOU AND ANORA HEALTH, YOUR PROVIDER, AND CERTAIN OTHER ANORA Health PARTNERS TO RESOLVE DISPUTES WITH EACH OTHER ON AN INDIVIDUAL BASIS THROUGH FINAL AND BINDING ARBITRATION. Welcome to Anora Health. By accessing and utilizing our mental health services, including but not limited to evaluation and treatment, you agree to the following Terms and Conditions. Please review them carefully.
- Payment Terms
- Invoices and Receipts: All invoices and receipts for payments will be sent to the email address provided by the patient. It is the patient’s responsibility to ensure that the email provided is accurate and up-to-date.
- Authorization of Payment: By using our services, you authorize in good faith all payments made using your credit card or debit card. You affirm that you are an authorized user of the provided payment method.
- Responsibility for Fees: You agree to pay all fees associated with the evaluation and treatment of mental health services provided by Anora Health. This includes but is not limited to:
- No-show fees
- Evaluation fees
- Costs of laboratory tests
- Any additional fees for services rendered
- Insurance Policy: At this time, Anora Health does not accept insurance for covering the cost of ADHD evaluations. It is at the sole discretion of each individual to choose whether they wish to continue with treatment under these terms.
- Provision of Terms: These Terms and Conditions will be provided to you in a timely manner to ensure transparency and understanding.
- Cancellations and Refunds
- Cancellation Policy: If you need to cancel or reschedule an appointment, we request that you notify us at least 48 hours in advance. Failure to provide adequate notice may result in a no-show fee.
- Refund Policy: All purchases made for services provided by Anora Health are final. Once payment has been processed:
- No refunds will be issued under any circumstances.
- All fees are non-refundable, including but not limited to payments for evaluations, treatments, laboratory tests, and no-show fees.
- This policy applies regardless of the reason for cancellation, dissatisfaction with services, or changes in personal circumstances.
- Agreement to Terms
By engaging with Anora Health’s services, you acknowledge that you have read, understood, and agreed to these Terms and Conditions. Your continued use of our services constitutes your acceptance of these terms.
